Step 1
1. Download and Fill
Download the Sample Submission Form. Fill in all the necessary fields. Print the Sample Submission Form. This is the ticket to get your samples moving forward. We will not be able to process your sample without it!
STEP 2
2. Submit Form
Fill in the form below and don't forget to upload your Sample Submission Form. Please make sure to put in the correct Courier and Tracking ID/URL information to ensure your samples are processed in a timely manner.
Note: that the information we collect on farms is kept confidential. Any metadata collected and sequencing data generated are used to improve our in-house pathogen database and to help better serve growers.

Step 3
3. Print and Pack
Print the Sample Submission Form to send with the samples. Pack it in with the samples, but make sure it's protected from the ice or dry ice.
And that’s it! Once you send it, please reach out to your representative to let them know it’s on the way.
Please note that it is important to fill in all the required fields correctly in the forms or we may have to hold and delay your sample to get the proper information. In the case of no communication, we will hold samples for a week maximum before discarding.
